To take a history in faster way. Since it's called history, we make it a story, so it's easy la..

1. Patient's profile, same

2. Chief complaint, same

3. HOPI, draw a horizotal line on ur paper, make it as a timeline. Describe 1st symptoms he had from the left + Rx he received, whatever related. Move to the right that is what happen today
Don't forget to ask history about risk factor, aetiology, differentials and complications ( In order to do that, ilmu kenela cukup beb)

4. Other history tu cam biase la

5. Don't forget to ask what patient concern about his disease (just to show u have empathy and for counselling.

6. For examination, no need to write one by one or try to remember. Just draw a man and u can just tick sign yang die ada, and X kat signs yang he don't have.

Tryla, I think it is useful. So, u won't miss any history and don't have to look back nervously in ur notes if u were to asked something about ur history.
